CONSENT FORM FOR HANDLING PERSONAL INFORMATION

ABOUT THIS CONSENT FORM

PeakPersonality is committed to protecting the confidentiality, integrity, security, and privacy of

personal information entrusted to us. In fulfillment of this dedication, we adhere to our obligations

concerning the collection and use of Personal Data, as well as uphold your rights, all in accordance

with the General Data Protection Regulation ("GDPR").

WHAT PERSONAL DATA WILL BE COLLECTED?

The Personal Data collected will include:

  • results of a personality test;

  • names;

  • contact information.

 

FOR WHAT PURPOSES WILL MY PERSONAL DATA BE PROCESSED?

The Personal Data mentioned above will be exclusively processed and analyzed with the sole

purpose of gaining a profound understanding of your character and your dynamics within a group.

This process is intended to facilitate personalized insights, enabling us to assist you in enhancing your

overall performance.

HOW WILL MY PERSONAL DATA BE PROCESSED?

PeakPersonality will protect your Personal Data with specific safeguards, including

  • limiting access to specific individuals who are obligated to keep the information confidential

  • using security measures to avoid unauthorized access or data breach.

 

FOR HOW LONG WILL MY PERSONAL DATA BE PROCESSED?

The processing duration extends for the period of one year after the date of collection. After the

expiration of the contractual period, character test results will be stored in anonymized form.

WHAT RIGHTS DO I HAVE?

It is an important part of protecting your privacy that you always retain certain rights in relation to the Personal Data you provide us with. Your rights are:

  • The right to withdraw your consent. However, your withdrawal of consent has no effect on the lawfulness of data processing that occurred prior to your withdrawal (art. 7(3) GDPR);

  • The right to access your Personal Data that is held about you by PeakPersonality;

  • The right to correct or update your Personal Data if it is inaccurate;

  • The right to request the deletion of your Personal Information;

  •  The right to limit the collection and use of your Personal Data under certain circumstances, as outlined in art. 18 of the GDPR (for example if you think your information is inaccurate);

  • The right to receive your Personal Data in a structured, common computer format for your own purposes or for transferring it to another party;

  • The right to lodge a complaint against PeakPersonality regarding data protection issues with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
